Maintaining current contact information helps the board and its committees reach owners and tenants with important board announcements as well as during emergency events such as a water/gas main break or to report storm damage to vehicles or other property.  Each owner and paying tenant should complete a separate form.  Non-resident owners should enter their address at the end of the form.  Note: You do NOT need to have a Google account in order to update and submit the form.

Jan. 11 (2 p.m.) Update on Vaccine Rollout
Online Vaccine Registration Now Available For 1a and 1b Populations

There are two steps to schedule a COVID-19 Vaccine Appointment Online:

STEP 1: COMPLETE PRE-SCREEN AND VACCINE APPOINTMENT QUESTIONNAIRE
The Pre-Screen provides us with basic contact information and confirms your eligibility for vaccine, and will only take moments to complete. Once your eligibility is confirmed, you will complete a Vaccine Appointment Questionnaire. The questionnaire will take about 10 minutes to complete and you will need to be prepared to share: 
  • Contact information
  • Demographics (birthdate, etc.)
  • Answers to some medical history questions. This ensures you do not have any medical issues that would give reason to not be vaccinated
  • An active email address

STEP 2: APPOINTMENT SCHEDULING
Within a few days, the Health Department will call or email you to schedule an appointment.
